ID Requirement

DocuSign asks that each client prepare a valid ID for verification. Acceptable forms of identification include:

• State Driver’s License
• Canada Driver’s License
• US Passport
• US Military ID Card
• Inmate ID Card
• Other State Identification card
• Foreign Passports stamped by USCI

***If no such ID is available and cannot be reasonably obtained, the signer may have TWO CREDIBLE WITNESSES WITH VALID IDENTIFICATION PRESENT who know you and can attest to who you are UNDER OATH.***
